Ontario Health Teams (OHT) are multi-organizational, multi-disciplinary, health teams organized to deliver coordinated care in a geographic region with the goal of achieving the quadruple aim. The provincial government has made it a requirement that OHTs include First Nations

Overview

FNDHO is working to support First Nations who choose to participate in their Ontario Health Team models, working in collaboration with provincial partners as part of the evolving Ontario Health Teams environment.  Adoption and use of digital health tools, access to provincial systems and data, and improved client-centric workflows are being prioritized
