HCC Digital Health Expansion

The purpose of this initiative is to support all FNIHCC-funded HCC teams to implement and sustain the use of the digital health tools they need on their digital health journey.  We are currently in the second round of submissions for those who are interested.  Please contact info@fndho.ca for further details, or fill out our survey so that we may understand your needs and follow up with you – don’t forget to leave a contact name and number!

Virtual Care – Removing Barriers and Improving Access to Care

NOHCCN

This initiative is in place in order to establish Virtual Care processes, tools and resources for rural and remote First Nations Home and Community Care Programs.  FNDHO is currently piloting this initiative with four communities in the North Ontario Home and Community Care Network.

Maamwesying North Shore Community Health Services

This initiative is working to establish a process, tools and resources to 7 North Shore First Nations Health Centres to enable digital health tools for remote COVID Pre-Screening, COVID Screening and Testing Referrals, COVID At-Home Monitoring, and COVID Education Support and Coaching using the APP Vivify with Maamwesing North Shore Community Health Services

 

Enabling First Nations Health Teams’ Access to Provincial Digital Health Systems

ConnectingOntario Viewer Access Model

This initiative is in place to connect First Nations Healthcare Providers to core provincial digital health systems to improve circle-of-care coordination.  Currently, FNDHO is piloting this with 4 clinical teams working with First Nations Health Services

FNIHB Ontario Region Nursing Digital Health Access 

FNDHO is working with FNIHB Nursing to address circle of care issues between First Nation and Federal Service Providers.  FNDHO is currently piloting initiative with FNIHB nurses who are interested in participating.

 

Information Management using Digital Health Tools

SLFNHA Regional Health Information Management

FNDHO is working to enhance health information sharing among all circle-of-care providers and is currently working with SLFNHA on the initial requirements gathering stage of this initiative.

Digital Health Tools and Processes for Jordan’s Principle

FNDHO is currently developing requirements and standards for Digital Health tool support and is currently working on an Information Management Requirement Analysis as part of phase 3 of initiation/engagement for this initiative.  If you are part of a First Nations Health team and are interested in participating, please contact us at info@fndho.ca or fill out the survey.
